An opportunity has arisen for an enthusiastic and experienced health practitioner to join the Homefirst Team. This position is open to a Physiotherapist, Occupational Therapist or Nurse who has experience of working in the community or hospital settings and wishes to develop their assessment and rehabilitation skills.
You will be supported to develop whilst being part of a dynamic, progressive, multi-disciplinary team. The Team in Mid-Cornwall work closely with partnership organisations to support the intermediate health needs of our local population. The team are key to supporting discharges from hospital settings and preventing admission to hospital. You will be responsible for the assessment, planning and implementation of reablement and care. You will assist in the interpretation of relevant information, identifying problems and needs of a person and setting realistic goals. You will have excellent organisational skills combined with a wide knowledge base and experience of working with older persons, the community or hospital services.
Up to permanent full-time hours are available, a fixed term contract to try out the role or a secondment opportunity can all be explored.

Main duties of the job
To provide a high standard of Occupational Therapy, Physiotherapy and Nursing skills to the service, performing specialist assessments of patients including those with highly complex presentations, and from this formulating a diagnosis and treatment and/or rehabilitation plan.
To operate as a specialist practitioner being first contact for urgent requests to help both prevent hospital admission and facilitate early discharge.
To utilise specialist skills in community assessment and management of the acutely sick patient.
To provide leadership for support staff and junior staff through supervision, training and appraisal.
To participate in the planning and development of services within the designated team.
Model and deliver excellent evidence based clinical care, working as an autonomous Practitioner.

Education & Qualifications
Degree or diploma in Occupational Therapy, Physiotherapy ,Nursing
HCPC registered, NMC Registered
Evidence of appropriate postgraduate courses and CPD to support practice, to degree equivalent level
Knowledge and Abilities
Confident and competent leadership with the ability to work in diverse and complex situations
Proven leadership qualities, including the ability to motivate a team and negotiation skills
Awareness of safeguarding policies and procedures
Skills and Aptitude
Clear understanding of clinical effectiveness and the need to provide evidence based care
Teaching, Assessing, coaching and mentoring skills to facilitate an effective learning environment for staff and students
Substantial post registration clinical experience in a similar role
A broad range of clinical experience in a variety of community and inpatient settings
Extensive experience of working within a multidisciplinary team
Experience of managing the risks associated with working in a lone working environment
